Request to Attend Traffic Violator School

I understand that I may be granted permission by the court to attend traffic violator school (TVS) if I have not attended TVS for any moving violation occurring within 18 months of this citation anywhere in California and I was not cited for going more than 25 miles over the posted speed limit.

My court notice shows that I appear to be eligible to attend (TVS) and I would like to attend.  I hereby enter a plea of GUILTY or NO CONTEST, if one was not previously entered.  I understand that I have the right to: an attorney (at my own cost); a court trial within 45 days from the date of the entry of this plea; confront and cross-examine witnesses against me; present evidence in my own behalf and avoid self-incrimination. 

I understand that I must meet the following conditions to request and successfully complete Traffic Violator School (TVS):

• My request for approval to attend TVS is being made before the due date on my court notice. 
• I understand that there is a $54.00 non-refundable traffic school fee that must be paid to the court.
• I understand that after the court approves my request to attend TVS, I must pay the traffic fine in full plus the $54.00 fee to the court before attending TVS.
• I understand that I will have 60 days from the original due date on my citation to complete TVS.
•  I understand that each traffic school also charges a fee for the course and I am responsible to pay the fee directly to the school as well.

Upon successful completion of TVS, this citation will remain confidential on my DMV driving record.  By submitting this form, I agree to successfully complete TVS and pay the fine plus the $54 traffic school fee to the court.  After the 60-day deadline has passed, if the court has not received verification that I have completed TVS or paid the fine and fees, I understand that the charges on my citation will show as convictions on my driving record.
